Role Description
This listing is for full-time, part-time, or PRN Radiologic Technologist at Haskell Memorial Hospital. The role requires some weekday and weekend on-call. The Radiologic Technologist will be responsible for conducting radiologic procedures, ensuring radiation safety, and working closely with the medical team to provide high-quality imaging services.

Qualifications
- Radiologic Technology and American Registry of Radiologic Technologists (ARRT) certification
- MRI experience required
- C-arm experience preferred
- Experience in radiation safety and medicine
- Knowledge of radiology procedures and equipment

Chronic Care Management Coordinator Assistant

Position Summary
Role Description
This is a full-time position. The Chronic Care Coordination Assistant will work collaboratively and under the direction of the Chronic Care Nurse Manager to provide disease management and healthy lifestyle coaching, education, training, and motivational support to patients. This position is accountable for working with the Chronic Care Nurse Manager and representing our hospital and clinic across multiple constituents.
Essential Duties and Responsibilities
Exhibit competence in following the directions of the RN Nurse Manager to address:
- Health Management
- Comprehensive Assessment and Care Planning
- Interpersonal Communication
- Education/Coaching on health issues as directed by RN Nurse Manager
- Health insurance and benefit knowledge at entry level
- Community Resource knowledge
- Research and Evaluation skills.
- Provides outreach, health management, education, and other resources to patients with chronic health conditions, and access to referral sources and community resources.
- Supports the patient with coaching and educational needs by focusing on what is most important to their quality of life under the guidance and direction of the RN Nurse Manager.
- Connects with relevant community and/or healthcare resources to support solutions; provide educational tools, and promote self-management.
- Collaborates with the patient to track progress interventions and goals.
- Conducts health and wellness encouragement and education as directed by the RN Nurse Manager to assist participants in making lasting positive changes for health and wellness.
- Monitors patient’s progress toward his or her optimal level of wellness, communicates variances to the RN Nurse Manager, documents and tracks time in the Cross TX, the Chronic Care Management coordination platform.
- Promotes wellness and provides education regarding preventative care measures.
- Acts as a liaison between referral sources, facilities, and outside entities to prevent and/or resolve continuity of care issues.
- Communicates with service partners, Primary Care Providers, care coordinators, and other health professionals to provide care coordination and ensure the plan of care includes efficient use of resources.
- Demonstrates skills in effectively coordinating and monitoring care to promote quality and cost-effective outcomes.
- Proactively follows up with patients discharged from all hospitals, rehab facilities and emergency rooms to recruit patients into the Chronic Care Management program, ensuring the patient has an appointment with their provider, and reviewing for unmet needs prior to the upcoming appointment.
Education and/or Experience
- High school diploma or equivalent
- Coaching education and training preferred.
- Previous clinical experience including working with the geriatric population is preferred.
- Previous work experience with patient engagement, patient coaching, and patient education is preferred.
